Book direct at museivaticani.va 30+ days ahead — the official reservation adds only €5 to the ticket price and lets you skip the outside line (which can be 2+ hours in June-August). For the Sistine Chapel with fewer crowds, book an early-access guided tour that enters before public opening — significantly more expensive but the room is nearly empty.
